This often occurs when the minute hand falls off and is repositioned on the square arbor facing in a different direction than it did before.

The solution is to move the hand further around the dial so that the clock may chime once every quarter.

After the hourly striking of the clock, remove the minute hand and place it on the square arbor pointing towards the number “12” on the dial.
